I am trying this for the second time, last time was a flop!!
1- Do I have to make sure that the equipment is free from grease, like with RI?
2- Which attachment do I use on the mixer?

I have made it and I do like it best for cookies. It's great for shiny cookies. (see my 1st cookie bouquet I made this week)
Make your cookies.
Decorate and let them sit on cookie sheets in your oven away from looky looers. That way, they won't be bothered. Let them sit over night and they will be dry. Once in awhile I would turn the heat up and then shut it off to speed their drying. I also heard leaving the oven light on works. I do that too.
PS-the base of the cookies were done with Toba's glaze.
The decorating was done with RI.
No, the mixer does not have to be grease free. I have a Bosch Universal and it only has a plastic bowl. I used the whisk attachment.
I didnt follow the order like I should have because Ive heard that if you use all of the milk, it will be too thin. I added the corn syrup to the powdered sugar, mixed that up then streamed in the milk til it was the consistency I needed for details. I thinned it out a bit more for flooding, but I never used all of the milk it called for. I dont know if this is the RIGHT way to do it, just know it worked fabulously for me!
